Among the human factors addressed by CRM, communication failures and inadequate assertiveness are the most persistently identified contributors to accidents in commercial aviation. The FAA's Risk Management Handbook (FAA-H-8083-2) makes clear that even technically proficient pilots operating perfectly serviceable aircraft have been lost because a crew member who recognized the hazard could not — or did not — communicate that recognition in time to prevent the outcome. Understanding not just what CRM communication principles are, but how they work and why they are structured the way they are, is essential knowledge for any Airline Transport Pilot certificate candidate.
The Communication Loop: Structure and Failure Modes
All cockpit communication follows the same basic model: a sender encodes a thought into words or signals, transmits through a channel (voice, interphone, gestures, written checklists), and the receiver decodes what arrives. The deceptively simple flaw in this model is that meaning is encoded by one brain and decoded by a different brain, under conditions that may include loud engine noise, high task saturation, time pressure, fatigue, or cross-cultural differences in communication norms. Any of these factors can cause the decoded message to differ substantially from the encoded one — and neither party may be aware the gap exists.
The antidote described throughout FAA CRM doctrine is closed-loop communication. In a closed-loop exchange, the receiver reads back or paraphrases the message, and the original sender explicitly confirms that the readback was correct before the loop is considered closed. This two-step verification is the mechanism behind standard ATC readbacks, checklist challenge-and-response, and critical CRM callouts like altitude alerts and approach briefing items. The confirmation step is not a courtesy — it is the functional close of the loop. A readback that goes unverified by the sender leaves the loop open and the potential for miscommunication intact.
Standardized phraseology and structured briefings — departure briefings, approach briefings, threat-and-error management briefings — serve a related purpose: they establish shared mental models before high-workload phases of flight. When both crew members have explicitly agreed on the plan, altitudes, callouts, and abort criteria, deviations from that plan become immediately detectable by either seat. Silence or a mismatch between the briefed plan and actual events becomes a communication cue in itself.
Authority Gradient: The Hidden Communication Barrier
The most thoroughly documented communication hazard in multi-crew operations is the authority gradient — the perceived power differential between the pilot-in-command and other crew members. A steep gradient exists when one crew member's status, experience, reputation, or behavior discourages others from voicing concerns. Classic accident case studies reviewed in CRM training programs demonstrate that crew members sometimes recognized a developing hazard well before impact but phrased their concern so indirectly — or abandoned the concern after a single dismissal — that the captain never understood a safety-critical objection was being raised.
The FAA's Risk Management Handbook and Advisory Circular 120-51 (Crew Resource Management Training) both describe the steep authority gradient as a systemic hazard independent of the captain's actual competence. Experience and authority in the left seat are legitimate and necessary; an atmosphere in which that authority cannot be questioned is not. The inverse problem — an overly flat gradient where authority and structure are constantly challenged — introduces its own confusion and workload. The CRM goal is a balanced authority gradient: the captain retains clear command authority, but the crew environment is explicitly and actively structured so that any crew member can and will raise safety concerns without fear of ridicule or punishment.
Achieving this balance is a shared responsibility. The captain sets the tone through demonstrated openness to input; first officers fulfill their duty by raising concerns even when doing so is uncomfortable. FAA CRM doctrine frames this as a professional obligation, not a personality trait.
Assertiveness: Structured Escalation in Practice
Assertiveness in CRM is defined precisely: it is the ability to state a concern clearly, professionally, and with enough persistence to ensure it is genuinely received and addressed. It is explicitly not aggression, insubordination, or emotional confrontation. The FAA describes a structured escalation pathway for situations where an initial concern is not acknowledged:
- Probe: Ask a question that surfaces the issue without direct accusation. This approach is less threatening and often sufficient. Example: "Captain, I'm showing 4,200 pounds of fuel — do you want to re-examine our alternate selection?"
- Alert: State the concern directly and specifically if probing does not resolve it. Vague language — "I'm not sure about this" — can be easily dismissed. Specific language — "Captain, I'm concerned our current fuel load does not meet the regulatory minimum to reach the filed alternate" — cannot. The specificity forces engagement.
- Challenge: If the concern has been stated directly and still not acted upon, restate it with supporting data: instrument readings, calculated numbers, regulatory requirements. Reference the checklist, the chart, or the regulation by name. The goal is to make it harder to dismiss the concern than to address it.
- Emergency warning: If safety of flight is imminent and previous steps have failed, the concern must be framed explicitly as an emergency requiring immediate crew decision or action. This step should include the other pilot's name, direct eye contact, and an unambiguous statement of the safety consequence.
In extreme cases — including suspected incapacitation, clear regulatory violation, or imminent controlled flight into terrain — CRM doctrine supports a crew member taking appropriate corrective action, consistent with the general emergency authority and responsibilities of the pilot in command under 14 CFR 91.3. The expectation of such action is embedded in multi-crew operations training standards and operator procedures required under 14 CFR Part 121.
Memory Aid
Many operator CRM programs use the mnemonic PACE to encode this escalation sequence: Probe, Alert, Challenge, Emergency warning. PACE is an industry/operator-developed training mnemonic rather than a term explicitly defined in AC 120-51 or FAA-H-8083-2, though the escalating-assertion concept is consistent with FAA CRM principles described in those documents. Individual operators may use different terminology, but the escalating-commitment framework is universal.
Communication Barriers: Fatigue, Culture, and Stress
Beyond authority gradient, the FAA identifies several other communication barriers that ATP-level crews must recognize and actively manage. Fatigue degrades both the encoding and decoding ends of the communication loop simultaneously: a fatigued sender may believe they transmitted a complete message when they did not, and a fatigued receiver may miss an abnormal callout entirely while believing they heard it correctly. CRM addresses fatigue not simply as a performance issue but as a communication hazard that changes the threshold for misunderstanding.
Cultural norms around hierarchy, directness, and conflict avoidance vary substantially across nationalities and organizational cultures. A crew member whose background includes strong deference to authority figures may not escalate even when they understand that CRM demands it. Multicultural crew training and explicit discussion of these differences during initial and recurrent CRM training is specifically addressed in FAA guidance.
Stress and high workload narrow attention. Under stress, crew members tend to revert to the communication habits most deeply ingrained — which may not be the standardized, closed-loop habits trained in the simulator. This is one reason CRM training emphasizes deliberate, repeated practice of communication techniques under realistic scenario pressure rather than passive classroom review.

- An unverified readback is treated as complete. If a question describes a receiver reading back a clearance and the sender moving on without confirming, the loop is not closed — that is a CRM communication failure.
- Dominant captain behavior is labeled "strong leadership." The FAA frames a cockpit where the captain's word is unchallenged as a hazard. Experience does not eliminate the need for crew input.
- Fatigue is treated as only a performance issue. The correct framing includes fatigue as a communication barrier that degrades both sending accuracy and receiving accuracy simultaneously.
- CRM applies only in emergencies. CRM communication discipline — closed loops, structured briefings, standard callouts — is practiced continuously on every flight, not reserved for abnormal situations.
